I would like to welcome you again for the upcoming season. It will be here before you know it. In an effort to better serve our student-athletes, all physical forms are posted online under the appropriate heading. Please click on one of the three links below. If you are a returning athlete to WSU then please click on that link below, fill out the Annual Health Questionnaire as well as the Contact and Insurance form and return it to the address listed below. If you are a new athlete, please click on that link and download the forms. They too should be returned to the address listed below. It is mandatory that the contact info be filled out and updated insurance information be done each year. We also need an updated copy of the front and back of the insurance card. The athlete will not be cleared without this information, so we encourage you to complete and return the packet prior to the start of the school year. Also, there have been a couple of changes to the insurance policy so please read it carefully.
